How to Claim Your Rebate
To claim your rebate, simply follow these steps:
- Ensure your Extras Cover includes Healthier Lifestyle benefits.
- Obtain a Health Management Form signed by your medical provider, such as a doctor or physio, confirming that your gym membership is part of a health management program.
- Submit the completed form along with the receipt from either Carbon Gym or ETC as a claim through the Nib or GU Health online portal or app.
Remember, the Health Management Form is valid for two years from the first time you claim. After that, a new form will be required to continue claiming for this service.
